I cannot attach my serial number to my account. What do I do?

  1. If you have a home kit in front of you, carefully and with clean hands open your home kit.

  2. Locate the biohazard bag. Inside the biohazard bag is your Collection Tube.

  3. On the Collection Tube, there is a nine-digit serial number that starts with “SHK” or “NML”.

  4. Login to your ASMO account at https://portal.switchhealth.ca

  5. On the left-hand side, click “Home Test”

  6. Click “Continue”

  7. Enter the 9-digit serial number located on the Collection Tube. Note, ASMO will automatically add the “-“ or dashes in your serial number.

IMPORTANT: If you do not assign your serial number to your patient account on the Switch Health portal, we will be unable to process your results.
